  • The Monitoring and Evaluation (M&E) program was implemented in October 2023 and serves the needs of youth in the child protection/foster care systems as their placement is determined.  The program is intended to be of short duration lasting from two weeks to a maximum of 28 days.  Youth who are not discharged or placed by the 28th day of residence are automatically enrolled in the Summit Youth Academy behavioral health program.  The M&E program clinician/coordinator administers several assessments to include The Child and Adolescent Needs and Strengths (CANS), Youth Comprehensive Risk Assessment (YCRA), Risk of Sexual Victimization/Perpetration Screener, Trauma Symptom Checklist for Children (TSCC) and clinical interviewing. and direct care staff observations on how the youth interacts with his peers, authority figures and day-to-day living.  These assessments and observations will aid in the evaluation process to determine whether the youth’s needs are intense residential treatment, intense outpatient treatment, or outpatient care.  During the evaluation period the youth is provided with counseling services, attends psychoeducational groups, and participates in all daily activities with his assigned group.  He is enrolled in public school and scheduled for medical, dental, and vision services as needed.  The clinician who coordinates the program attends a weekly staffing to which the youth’s case worker, juvenile probation officer, and guardian ad litem are invited.  The meeting is being facilitated by the Department of Health and Welfare Congregate Care team.  During these staffings, the clinician reviews the youth’s assessment data, updates the group on the youth’s behavior, and makes recommendations for the best course of treatment.  The clinician writes a discharge summary with recommendations if it is determined that the youth will discharge to his family home or another placement.  The discharge summary is sent to the Congregate Care team.  If it is determined that the youth will be admitted to the Summit Youth Academy behavioral health program, a Summit Youth Academy case manager will be assigned as point of contact, and the clinician will continue to see the youth for counseling services.
